Summary

This Hematology/Oncology Nurse Practitioner will provide services to Veterans at the Erie VA Medical Center and surrounding community based outreach centers.

Duties

• Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.- Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with the interdisciplinary team
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

• Responsible for oversight of patients in treatment room receiving chemo and immune therapies
• Manage all reactions to chemotherapy and biotherapy treatments escalating to conditions as needed
• Perform bone marrow aspirations and biopsies
• Sign and amend chemotherapy orders as needed - for toxicities - blood counts
• Order blood transfusions to be administered on 9 west for patients with chronic transfusion needs
• Provide case management for complex oncology patients to assess - monitor - and evaluate care for chemotoxicities - comorbidities - and psychosocial issues
• Provide ongoing evaluations for care management patients - work closely with attending physicians - fellows - Primary Care - and other specialties Travels to other Western Market sites and community based outreach centers as needed for close to me infusion oversight
