What You'll Do
As a Care Manager, you will serve as a trusted advocate, ensuring individuals receive the support and care they need. Your primary responsibilities include:
Conducting comprehensive assessments to understand individuals' medical, social, and personal needs.
Developing a person-centered Life Plan that integrates clinical and non-clinical services.
Coordinating care with healthcare providers, community organizations, and support networks.
Addressing barriers to care, including social determinants of health.
Facilitating referrals to necessary medical, mental health, and social services.
Providing health promotion and wellness support to help individuals achieve their goals.
Ensuring all services are culturally and linguistically appropriate.
Advocating for individuals and empowering them to make informed choices about their care.
Maintaining strict compliance with HIPAA regulations and other healthcare standards.
Keeping accurate documentation and updating care plans as needed.
What You'll Need
To be successful in this role, you must meet the following qualifications:
Education & Experience:
Bachelor's degree with two years of relevant experience OR
Registered Nurse (RN) license with two years of experience in any field (not limited to case management) OR
Master's degree with one year of relevant experience.
Language Skills: Fluency in Spanish (Required).
Skills & Competencies:
Strong time management and organizational skills.
Ability to work both independently and collaboratively within a team.
Experience with case management or care coordination is preferred.
Proficiency in using technology and health records systems.
Additional Details
Work Setting: Hybrid remote, with some in-person visits required.
Location Requirements: Candidates must be based in Rochester, NY, or be willing to relocate before starting.
