JOB SUMMARY Under the direct supervision of the Clinic Manager, provides skilled nursing care to participants. This position is responsible for working directly with the Interdisciplinary Team and participant family members or caregivers to achieve the goals of the care plan. Communicates to the family or caregiver and provides appropriate patient education. Promotes professional working relationships with both internal and external customers. Adheres to and supports all organizational policies and procedures and standards. Promotes teamwork!
ESSENTIAL DUTIES AND RESPONSIBILITIES Organizational (Core Rating Factors): - Ensure discretion with confidential information.

- Demonstrates appropriate level of time management in support of co-workers and the entire team.
Departmental (Job Specific Tasks): The principle duties of this position include, but are not limited to, the following:
- Must maintain calm and professional demeanor.
- Performs blood draws and other laboratory testing, medical management, IV therapies, wound care, etc. as directed by medical providers.
- Documents test results and examinations in the EMR.
- Implements orders written by medical providers.
- Provides medication reconciliation and communicates with facilities and the pharmacy.
- Communicates effectively to the IDT and to family and caregivers, including providing instructions regarding medications and treatment.
- Performs phlebotomy functions for participants when necessary. Identifies and labels specimens appropriately.
- Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians
- Follows all PACE of West Baltimore Policies and Procedures and OSHA safety guidelines.
- Maintains the confidentiality of all company procedures, results, and information about participants, clients, or families. Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions.
- Maintains professional and courteous behavior on the job. Ensures customer satisfaction. Will support the success of the entire healthcare team.
- Maintains safe working environment. Follows PACE of West Baltimore Safety Policies and Procedures.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Performs other duties as required or requested in a positive and helpful manner to ensure a smooth-running work area.

QUALIFICATIONS - Graduate of an accredited Registered Nurse Program and licensed as a professional in Maryland is required.
- One year of health care experience in hospital, long-term or home health care setting with 1 year of experience with a frail or elderly population is required.
- Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.
- Must submit to and pass a pre-employment drug/alcohol screening and criminal background check.
- Must have a current Driver’s License and driving record must be in compliance with Company policy. Must maintain current automobile insurance.
- Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records.
- Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public.
- Must be free of communicable diseases that can be readily transmitted in the workplace. TB test is required within first 7 days of employment. Hep B vaccinations are offered but not required.
- Operates in full compliance with all applicable federal, state and local fire, health, safety, sanitation, Emergency procedures and other Standards prescribed in law or regulations.
- Knowledge of medical equipment and instruments.
- Knowledge of common safety hazards and precautions to establish a safe work environment.
- Skill in applying and modifying the principles, methods and techniques of practical nursing to provide on-going care for participants.
- Ability to perform phlebotomy functions.
- Skilled in identifying problems and recommending solutions.
- Skilled in preparing and maintaining records, writing reports, and responding to correspondence.
- Skilled in developing and maintaining department quality assurance.
- Ability to react calmly and effectively in emergency situations.
- Current Driver’s License, reliable transportation, a satisfactory driving record and proof of auto insurance.

- Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to within normal range, with or without reasonable accommodation. Must be able to communicate effectively in verbal and written form with all levels of personnel within and outside of the organization.
- Pressure Factor - Requires working under stressful conditions. Moderate pressure to meet scheduled and recurring deadlines.
