**Job Summary and Responsibilities** The Acute Care Therapy Department at St. Joseph's Hospital and Medical Center provides the evaluation and treatment of patients with a wide variety of diagnoses in a fast-paced, challenging, and rewarding medical setting. Our Therapists are critical members of a multi-disciplinary team dedicated to achieving optimal outcomes for our patients. We are committed to teamwork and collaboration; our participation in interdisciplinary patient rounding occurs daily on a number of units throughout the facility providing therapy professionals to work autonomously. Expectations: + The Physical Therapist provides patient care including evaluating patient for treatment, providing and directing administration of the Physical Therapy care plan upon Physician referral. Consults with other health care disciplines to co-ordinate therapeutic programs and discharge planning for individual patient. + Instructs patients and care givers in home programs for exercises and functional activities. + Assists with patient program management, while also delivering patient services. + Serves as a resource by sharing expertise and knowledge and assisting with staff professional development.
