Job Summary:
1. The role is a 13-week contract beginning on 2/7/25, paying $2,618 per week.
2. The candidate is required to have taken COVID vaccines + Booster and the 2024-2025 flu vaccine at the time of submission.
3. An active MA license is required at the time of submission.
4. The candidate is required to have a Master's degree in social work from an accredited school.
5. A minimum of 2 years' experience in healthcare is necessary, with a special interest and understanding of mental health diagnosis, substance issues, and interpersonal/domestic abuse.
6. The candidate must possess LCSW licensure with a plan for LICSW.
7. Strong interpersonal, organizational, and computer skills are required.
8. The role requires the ability to work collaboratively in interdisciplinary teams and to exercise sound clinical judgment in fast-paced and demanding settings.
9. The Clinical Social Worker will assist patients and their families with stress and difficulties related to illness, personal issues, financial difficulties, and other emotional/mental health issues.
10. The job involves collaborating with nurse care managers and other members of the multidisciplinary team to facilitate safe and effective discharge plans.
11. The role requires a passion for working with elders who may be more vulnerable due to advanced age or medical comorbidities and issues related to aging.
12. The candidate should have the ability to develop trusting relationships with diverse patients struggling with mental health and/or substance use disorders.
13. The Clinical Social Worker is responsible for performing biopsychosocial evaluations, documenting notes, planning and participating in family meetings, providing support and education to patients and families, facilitating the continuum of care, completing insurance reviews, and participating in daily rounds and consultations with other clinical staff.
